Our Doctors of Physical Therapy combine advanced training and experience with their deep compassion for people to provide patients with comprehensive treatment programs to restore pain free movement and function. To help speed up your recovery and maximize your improvement, we offer convenient, extended hours (7am-7pm M-F, and Saturday mornings) as well as same day new patient appointments.

If you have any insurance other than Worker's Compensation you will need to download and fill out the first three forms. If you are filing through Worker's Compensation, you will need to print and fill out all four forms. Click on the form you need to download an Adobe PDF document which you may print. Each patient's health insurance company is billed as a courtesy. We currently accept most private insurance. We are a MEDICARE certified facility and accept Worker's Compensation cases. In order to do so we must have a valid prescription written and signed by a physician.

When can I come to therapy?
Our office hours are Monday through Friday from 7:00 a.m. to 7:00 p.m. and Saturday 8:00 a.m. to 12:00 p.m. Appointments are made in advance. However, we reserve time each day for new patient referrals as physicians request and for our sports or work injury screenings. Please provide 24 hour notice if you need to cancel and reschedule.
How do I schedule an appointment?
Call our office at (434) 817-0980. Please have available your doctor’s referral and your insurance information. We offer evaluation and treatment within 24 hours of your call. Same day evaluation and treatment may often be available.
Do I need a doctor's referral?
The state of Virginia allows direct access for patients to receive a Physical Therapy evaluation without a physician referral. However, treatment by a physical therapist requires a diagnosis by a physician after 15 days. In general, insurance plans often require a physician referral to physical therapy for coverage of care. Thus, we recommend to all patients to see or contact their physician prior to starting formal physical therapy treatment. We can provide any inquiring patient with recommendations to leading physicians who are experts in their specialties for initial visits or second opinions. You can be evaluated by us without a referral, but you will be responsible for the cost of the evaluation
How do I pay for my physical therapy?
APTA accepts most private PPO insurance plans. For a comprehensive listing of insurance plans accepted, please see the Patient Information page under the heading Contracted Insurance Plans with APTA.
Should financial difficulties prevent you from seeking complete care, please let us know, we may be able to assist you. For accounts that are 45 days past due, a 1.5% interest rate will be added to your outstanding balance (18% annual).
What do I need to bring with me on my first visit?
- Doctor’s referral or prescription
- Any x-rays, MRI or operative reports you have related to your injury
- Insurance card and any claim forms, if required
- Comfortable clothes and tennis shoes
May I complete the paperwork in advance?
Sure, just visit the Patient Information Forms section above and click on the links to print out the forms. Complete them and bring them to your appointment.
What can I expect at my first visit?

How often will I need to attend physical therapy?
Most patients come to physical therapy one to three times a week. Your schedule will be dictated by your treatment goals and determined by your physical therapist based on the nature of your injury. The more committed you are to coming to therapy and to your home exercise program, the more satisfied you will be with your progress.
How long will each appointment take?
Plan to spend between 60 and 90 minutes in physical therapy. Our comprehensive programs take time to complete. Your total treatment time is sometimes longer or shorter based on your individual needs. Your therapist is responsible for determining those needs based on your evaluation and plan of care. You will not sit idle!
